This is a request to reserve the conference room 123. The primary use of this room is for University Libraries' faculty, staff, and student meetings of up to sixty (60) persons. Also, included in the primary use category are meetings when a University Libraries faculty or staff member is part of the meeting group and serves as sponsor of the meeting.
The next priority of use is for University teleconferences, which are booked by the Media Services Specialist. Continuing Education room fees apply. Finally, other University groups may schedule the meeting room 123.
These groups will not be allowed to schedule standing meetings for this room, and meetings may be held weekdays during the hours of 8:00 a.m. and 4:45 p.m. Use is restricted to room 123. Fees apply.

Guidelines for Room Usage *
The library sponsor and/or the key holder from outside groups are responsible for making sure the room use guidelines are followed. Please check the boxes below stating you understand the guidelines. Please check the box to the right indicate you understand these regulations.
Fees *
Teleconference and outside group fees are $25 for meetings lasting up to 4 hours and $50 for a full day meeting. An additional $25 fee will be assessed for use of the teleconferencing equipment.
Key *
The key may be picked up in the Library Administrative Office, room 202. In order to pick up the key an ID must be left, and the key must be returned to the Administrative Office before 5 p.m. A fee of $50 will be applied if key is not returned or lost.
Finish *
I have read and understand the above information regarding the charges and use of Cook Library, room 123. I am the person responsible for enforcing the room use guidelines and responsible for the room key.
