Reservation Request Form for Cook Library Room 123

This is a request to reserve Cook Library Room 123, a conference room seating 60 persons that is located at the south end of the first floor of Cook Library.

Guidelines for Cook Library Room 123 Use:

  • This form must be submitted at least three business days (Monday through Friday, excluding university holidays) in advance of your meeting.
  • Reservation requests must be made by a university employee. Student group advisors may reserve the room for student group use.
  • Meetings may be scheduled for the following times: Monday through Thursday, 8 am until 9 pm; Friday, 8 am until 5:30 pm; Saturday, 9 am until 5:30 pm; Sunday, 12:30 pm until 9 pm. Standing meetings will not be scheduled for this room.
  • There is no charge for use of the room; however, groups may be liable for damages, lost equipment, or cleaning costs.
  • Deliveries of food, equipment, or other items are the responsibility of the group reserving the room and such deliveries should be made during the time the room is reserved.
  • While the library provides a computer, data video projector, and Internet access, limited technical assistance is available. Call the Administrative Office (601.266.4241) or Access Services (601.266.4250) during evening and weekend hours to see if technical help is available.
  • The room must be left in good order. Chairs and tables must be returned to their original locations and the door locked.
  • Meeting attendees must enter and exit through the library’s public entrance located at the north end of the building.
  • Meeting attendees must abide by the library’s Acceptable Behavior Policy.
  • The room key should be returned to the Cook Library Circulation Desk immediately after the meeting concludes.

Please bring your university ID card to the Circulation Desk in the lobby of Cook Library. The key will be checked out to your personal library account. It must be returned to the Circulation Desk immediately after your event concludes.
