Form for Appeal of Library Charges Use this form for all University Libraries' charges. It is not to be used for lost material charges. You MUST complete the entire form for the request to be processed. Completing the form authorizes members of the Fine Appeals Committee to review library charges.

You will receive notification of the decision of the Appeals Committee via U.S. Post. If you do not receive a reply within three weeks, it is your responsibility to contact the Circulation Department at 601-266-4250 between 8 a.m. - 5 p.m., Monday - Fridays.
